DIAGNOSIS WORKSHEET
This is a hard sheet to fill out. You must take an objective look at your illness and be prepared to participate in your treatment. To do this, you need to know what you are up against. If you feel you are not up to this by yourself, ask a friend or family member to help.
What did the tests/biopsy show?
What exactly does that mean?
What is the doctor's opinion of my condition?
What parts of my body are affected?
Can it spread to other parts?
What are my options as you (the doctor) see them?
How do you plan to make me well again?
Explain all of the plans you have for my recovery.
What is the treatment? Be specific.
How long will it take?
What should I expect from treatment?
How will it change my daily routine?
How often and for how long are the individual treatments?
What test and procedures will be used to monitor my progress?
What current research is being done? Where is it being done?
Where else can I get an opinion? If the doctor says no other opinion is needed, he or she is wrong.
Do I need to see more than one doctor?
What other type of doctor should I see?
Would you consider working with a team of doctors?
